What is Lap assisted Vaginal Hysterectomy?

Lap assisted Vaginal Hysterectomy, also referred as Laparoscopically Assisted Vaginal Hysterectomy is a surgical procedure done to remove the uterus and ovaries through the vagina and a small incision made in the abdomen.

Why is it performed?

A  Lap assisted vaginal hysterectomy may be recommend in the following conditions:  

  • Fibroids
  • Abnormal uterine bleeding
  • Endometriosis
  • Overgrowth of any tissue in the uterus lining.

How is LAVH Performed?
The surgery of performed in a special operation room:

  • The patient is sedated by administering general anesthesia 
  • The doctor will insert a laparoscope in the abdomen through a key hole opening and pump carbon dioxide gas to inflate the belly. 
  • The surgeon will make small incisions on the abdomen to insert small metal tubes called trocars and microsurgical tools to perform the laparoscopy.
  • The camera present at the tip of laparoscope helps to capture the images and project them on the monitor.
  • The surgeon will separate the uterus by detaching the ligaments which attaches the uterus to other pelvic structures.
  • The ovaries and tubes are removed if required, by detaching them from the ligaments and blood supply.
  • The specimen will be retrieved through vagina by making a stab incision.
  • After the removal, the incisions are sewed by absorbable stitches.

What are the precautions that need to be taken after the procedure?

  • Avoid lifting heavy objects or strenuous activity for 4-6 days post-surgery.
  • Avoid driving for some time after the surgery till the one feels comfortable,
  • Avoid sexual activity until advised by your surgeon
  • Do not use tampons
  • If constipated, use laxative or stool softeners
  • You can continue with you daily activities provided you don’t over-stress yourself
  • Take plenty of rest 
  • Visit your surgeon or rush to emergency if you experience:
    • Heavy bleeding from the vagina
    • High fever (more than 100 degree Fahrenheit)
    • Sudden sharp pain in the thigh or calf region 
    • Foul smelling discharge from the vagina 

What are the benefits of LAVH over the traditional Vaginal Hysterectomy?

  • Incisions made are very small
  • It is very economical
  • Pain and discomfort after the surgery is  minimal
  • Recovery is quick compared to open surgery
  • Removal of ovaries and tubes are easier than the traditional vaginal hysterectomy.

What are the common risks/ complications of LAVH? 

  • Infection
  • Bleeding  
  • Damage to bowel and bladder
  • Adhesions and Hernia 
  • Thrombosis – DVT, PE
